Case Manager - follow-up and outcomes (remote)

Cedar Rook Community Partners runs community programmes across Indianapolis, and we need a Case Manager to lead our follow-up and outcomes. Flexibility and kindness are expected. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with quarterly visits in Indianapolis. • We reply to every applicant.
