Case Manager - follow-up and outcomes (remote)

Rivermark Field Housing is looking for a Case Manager. The job centres on follow-up and outcomes for rent assistance, and it would suit someone who likes steady, practical work. You would join a large organisation of about 598 people, working mostly from home. What you will do • Help with applications and appointments for rent assistance. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Savannah. • We will tell you what to expect at each step of the hiring process.
