Case Manager - follow-up and outcomes (remote)

We are growing our team at Clearpath Meadow Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for repair requests. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Des Moines. • We reply to every applicant.
