Case Manager - follow-up and outcomes (remote)

We are growing our team at Lumen Birch Care Services Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Des Moines. • We reply to every applicant.
