Case Manager - follow-up and outcomes (remote)

Summit Thistle Early Learning is looking for a Case Manager. The job centres on follow-up and outcomes for enrolment, and it would suit someone who likes steady, practical work. You would join a small organisation of about 37 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for enrolment. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with a monthly team day in Denver. • We will tell you what to expect at each step of the hiring process.
