Case Manager - follow-up and outcomes (remote)

Clearpath Bright Housing is a nonprofit housing provider in Detroit. We are hiring a Case Manager to take on follow-up and outcomes. Housing is our mission and residents are our neighbours. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Fully remote, with quarterly visits in Detroit. • Applications are reviewed on a rolling basis.
