Case Manager - follow-up and outcomes (remote)

Rivermark Sable Housing is looking for a Case Manager. The job centres on follow-up and outcomes for resident services, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in New Orleans. • Applications are reviewed on a rolling basis.
