Case Manager - follow-up and outcomes (remote)

Northwind Iron Learning Group is an education provider in Jacksonville serving students and families, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Jacksonville. • We reply to every applicant.
