Case Manager - follow-up and outcomes (remote)

We are growing our team at Nova Cross Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Jacksonville. • Applications are reviewed on a rolling basis.
