Case Manager - follow-up and outcomes (remote)

We are growing our team at Westbrook Birch Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Milwaukee. • We will tell you what to expect at each step of the hiring process.
