Case Manager - follow-up and outcomes (remote)

Fernwood Sparrow Health is a healthcare provider in Omaha with clinics and support teams, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Omaha. • We will tell you what to expect at each step of the hiring process.
