Case Manager - follow-up and outcomes (remote)

Northwind Lantern Property Management Group looks after rental homes across the Madison area. We are hiring a Case Manager to take on follow-up and outcomes. Maintenance and leasing staff share information every day. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for lease renewals. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Madison. • Applications are reviewed on a rolling basis.
