Case Manager - follow-up and outcomes (remote)

Westbrook Cobalt Learning Group runs classrooms and tutoring across the Fort Worth area. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 12 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for attendance records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with a monthly team day in Fort Worth. • Applications are reviewed on a rolling basis.
