Case Manager - follow-up and outcomes (remote)

Ironwood Stone Care Services is looking for a Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish.
