Case Manager - follow-up and outcomes (remote)

Highbridge Meridian Health Group is a healthcare provider in Reno with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Reno. • We reply to every applicant.
