Case Manager - follow-up and outcomes (remote)

Fernwood Ivory Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for patient scheduling, and it would suit someone who likes steady, practical work. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Charlotte. • Applications are reviewed on a rolling basis.
