Case Manager - follow-up and outcomes (remote)

Foxglove Sparrow Care Services is a social-care provider serving older adults and people with disabilities around San Jose. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with a monthly team day in San Jose. • We will tell you what to expect at each step of the hiring process.
