Case Manager - follow-up and outcomes (remote)

Bright Basin Health runs clinics and patient services around Fresno, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • Fully remote, with quarterly visits in Fresno. • Applications are reviewed on a rolling basis.
