Case Manager - follow-up and outcomes (remote)

Foxglove Summit Learning Group operates schools and learning programmes around San Antonio. We are hiring a Case Manager to take on follow-up and outcomes. Families are partners. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for school calendars. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with quarterly visits in San Antonio. • We reply to every applicant.
