Case Manager - follow-up and outcomes (remote)

Clearpath Thistle Early Learning is looking for a Case Manager. The job centres on follow-up and outcomes for classroom supplies, and it would suit someone who likes steady, practical work. Low staff turnover is a goal we take seriously. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for classroom supplies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with an occasional day on site in Raleigh. • We reply to every applicant.
