Case Manager - follow-up and outcomes (remote)

Silverline Vista Community Partners Group is a nonprofit in El Paso supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 154 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Fully remote, with an occasional day on site in El Paso. • We reply to every applicant.
