Case Manager - follow-up and outcomes (remote)

Westbrook Harbor Care Services is a social-care provider serving older adults and people with disabilities around Savannah. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Savannah. • We reply to every applicant.
