Case Manager - follow-up and outcomes (remote)

We are growing our team at Cedar Nest Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 941 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Chicago. • Applications are reviewed on a rolling basis.
