Case Manager - follow-up and outcomes (remote)

Summit Rook Housing manages affordable housing communities across Dallas. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease compliance. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • Remote-friendly, with quarterly visits in Dallas. • Applications are reviewed on a rolling basis.
