Case Manager - follow-up and outcomes (remote)

Fernwood Logistics operates schools and learning programmes around Dallas. We are hiring a Case Manager to take on follow-up and outcomes. Families are partners. What you will do • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Part-time, about 20 hours a week, afternoons. • Fully remote, with an occasional day on site in Dallas. • We will tell you what to expect at each step of the hiring process.
