Case Manager - follow-up and outcomes (remote)

Rivermark Vale Housing Group is looking for a Case Manager. The job centres on follow-up and outcomes for waiting lists, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Hartford. • Applications are reviewed on a rolling basis.
