Case Manager - follow-up and outcomes (remote)

Clearpath Ivory Civic Services Group provides shared administrative services to local governments around Toledo, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 124 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with a monthly team day in Toledo. • Applications are reviewed on a rolling basis.
