Case Manager - follow-up and outcomes (remote)

Redstone Union Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for electronic records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 52 people, working mostly from home. What you will do • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with a monthly team day in Boston. • We reply to every applicant.
