Case Manager - follow-up and outcomes (remote)

Ironwood Pike Early Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 155 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in San Antonio. • We will tell you what to expect at each step of the hiring process.
