Case Manager - follow-up and outcomes (remote)

Clearpath Yarrow Civic Services Group works with county and city offices around Phoenix, and we need a Case Manager to lead our follow-up and outcomes. Benefits and stability attract long tenure. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for permits. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with permits. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Phoenix. • We will tell you what to expect at each step of the hiring process.
