Case Manager - follow-up and outcomes (remote)

Amberfield North Early Learning Group is an early-learning provider in Chicago. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Chicago. • Applications are reviewed on a rolling basis.
