Case Manager - follow-up and outcomes (remote)

Nova West Health is a healthcare provider in Phoenix with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with an occasional day on site in Phoenix. • We reply to every applicant.
