Case Manager - follow-up and outcomes (remote)

Nova Pine Housing is a nonprofit housing provider in Columbus. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 30 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Part-time, about 16 hours a week, afternoons. • Fully remote, with an occasional day on site in Columbus. • We reply to every applicant.
