Case Manager - follow-up and outcomes (remote)

We are growing our team at Harborlight Keystone Care Services Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Savannah. • Applications are reviewed on a rolling basis.
