Case Manager - follow-up and outcomes (remote)

Fernwood Sable Community Partners is a nonprofit in Detroit supporting families with food, housing and job help, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 140 people, working mostly from home. What you will do • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Detroit. • We reply to every applicant.
