Case Manager - follow-up and outcomes (remote)

Highbridge Bright Community Partners Group is a nonprofit in Jackson supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Jackson. • We reply to every applicant.
