Case Manager - follow-up and outcomes (remote)

Foxglove River Community Partners runs community programmes across Hartford. We are hiring a Case Manager to take on follow-up and outcomes. We are small enough to know everyone's name. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Hartford. • Applications are reviewed on a rolling basis.
