Case Manager - follow-up and outcomes (remote)

Lumen Juniper Health Group runs clinics and patient services around Baltimore. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for referrals. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Baltimore. • We reply to every applicant.
