Case Manager - follow-up and outcomes (remote)

Ironwood Wren Civic Services is looking for a Case Manager. The job centres on follow-up and outcomes for constituent requests, and it would suit someone who likes steady, practical work. Public service means patience, paperwork and pride in the outcome. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for constituent requests. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Portland. • We reply to every applicant.
