Case Manager - follow-up and outcomes (remote)

Summit Meadow Property Management is looking for a Case Manager. The job centres on follow-up and outcomes for resident events, and it would suit someone who likes steady, practical work. You would join a small organisation of about 126 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident events. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with a monthly team day in Detroit. • Applications are reviewed on a rolling basis.
