Case Manager - follow-up and outcomes (remote)

Northwind Forge Care Services supports families and individuals in Toledo with personal care and coordination, and we need a Case Manager to lead our follow-up and outcomes. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for client records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Toledo. • We will tell you what to expect at each step of the hiring process.
