Case Manager - follow-up and outcomes (remote)

We are growing our team at Amberfield Sparrow Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Indianapolis. • Applications are reviewed on a rolling basis.
