Case Manager - follow-up and outcomes (remote)

Amberfield River Community Partners is a Tucson charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 22 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with quarterly visits in Tucson. • Applications are reviewed on a rolling basis.
