Case Manager - follow-up and outcomes (remote)

Stonegate Summit Property Management is a Indianapolis property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 882 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease renewals. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Fully remote, with quarterly visits in Indianapolis. • We reply to every applicant.
