Case Manager - follow-up and outcomes (remote)

We are growing our team at Northwind Basin Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 375 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Fully remote, with a monthly team day in Jacksonville. • We will tell you what to expect at each step of the hiring process.
