Case Manager - follow-up and outcomes (remote)

Bluepeak Timber Housing provides affordable homes and support services around Dallas, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for repair requests. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Dallas. • We reply to every applicant.
