Case Manager - follow-up and outcomes (remote)

Greenridge Loom Learning Group is an education provider in Buffalo serving students and families, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for school calendars. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Buffalo. • We will tell you what to expect at each step of the hiring process.
