Case Manager - follow-up and outcomes (remote)

Summit Clear Care Services provides in-home and community support in Madison, and we need a Case Manager to lead our follow-up and outcomes. We train and promote from within. What you will do • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 28 hours a week, mornings. • Remote-friendly, with a monthly team day in Madison. • We reply to every applicant.
