Case Manager - follow-up and outcomes (remote)

We are growing our team at Harborlight Rowan Health Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 29 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • Fully remote, with a monthly team day in Spokane. • We reply to every applicant.
