Case Manager - follow-up and outcomes (remote)

We are growing our team at Lumen Junction Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for food distribution. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Albuquerque. • We will tell you what to expect at each step of the hiring process.
